PROM DANCE & TICKET INFORMATION

All CCHS students must complete THIS STUDENT AGREEMENT FORM if purchasing tickets to the Prom dance. This form is linked here, the flier above, and via Rycor. Ticket purchases are non-refundable.

We do permit non-CCHS students to attend ourProm Dance as guests of our CCHS students. Only one guest is allowed per CCHS student. All CCHS students who wish to bring a guest are required to turn in a completed Guest Request Form, WITH PHOTO IDENTIFICATION OR STUDENT ID W/PICTURE OF GUEST attached. Forms turned in without photo identification will not be accepted. The Guest Request form is downloadable and is located below, or a hard copy can be picked up in Student Services. The deadline for approval of these forms in MAY 3RD at 2:00pm. Please see the form for specific guest parameters.
